Job Description
Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a per diem Registered Nurse for the Home Care and Hospice Care Access Department to work weekdays and occasional weekends as required.
Essential Functions/Responsibilities:
- Perform as a contributing member of the Care Access Center for Atlantic Visiting Nurse, by accepting referrals for a Home Care and Hospice.
- Partner with Case managers, social workers, physicians, family members and patients, accepting referrals and obtaining the most accurate clinical information, and physician documentation to complete a referral for a Home Care or Hospice admission.
- Must have regulatory knowledge of Home Care, and obtain documentation of the same.
- Must understand levels of care, and criteria for a hospice admission.
Minimum Education:
- Graduate of an accredited Nursing Program required
- Bachelor's Degree preferred
Minimum Experience:
- 2-5 years of Home Care and Hospice Experience
Licenses/Certifications:
- Valid NJ RN license
